Initial Appointment means First Appointment for that specific Treatment Category. If available, it must be booked before any Follow-Up appointment in the respective category.
You have until 24 hours prior to your scheduled time to reschedule or modify your appointments online or by phone. Fees will be waived if the appointment slot is filled.
In most cases, we can check eligibility however we encourage you to check with your insurance provider prior to your appointment.
